The examiner also evaluates the driver’s mental health, including conditions such as depression, anxiety, and attention-deficit/hyperactivity disorder (ADHD). If the driver meets the FMCSA’s health standards, the medical examiner issues a Medical USDOT card, which is valid for up to two years.
Who Needs a Medical USDOT Card?
All CMV drivers who operate a vehicle that weighs more than 10,000 pounds, transport hazardous materials, or carry more than 15 passengers must have a valid Medical USDOT card. This includes truck drivers, bus drivers, and other commercial drivers. The FMCSA requires drivers to carry them at all times while operating a CMV.
How to Obtain a Medical USDOT Card

It is important to note that if the driver has a medical condition that could affect their ability to operate a CMV safely, the medical examiner may issue a limited-term card. In this case, the driver must undergo additional testing or treatment and obtain a new card once their condition is under control.
How to Renew Your Card
Medical USDOT cards are valid for up to two years, depending on the driver’s health status and any medical conditions they may have. Drivers must renew them before they expire to maintain their CDL and continue operating a CMV.
To renew their Medical USDOT card, drivers must undergo a new medical exam with a certified medical examiner. If the driver’s health has not changed significantly since their last exam, they may be eligible for a two-year renewal. However, if the driver has a new or worsening medical condition, the medical examiner may issue a shorter-term card or require additional testing or treatment.
