What is an MIS Report For The FMCSA?
A helpful guide to understanding what MIS reports to the FMCSA are, what goes on them, and who is required to submit them

An FMCSA (Federal Motor Carrier Safety Administration) MIS report is a “Management Information System” report. Its purpose is to summarize a motor carrier’s annual drug and alcohol testing data for their employees performing safety-sensitive duties. This must be submitted to the FMCSA on request.
Understanding MIS Reports
Motor carrier companies must keep records detailing all drug and alcohol testing conducted on commercial drivers within a company. This also includes any positive results and the reason for testing. These records should be maintained for at least 5 years and kept in a safe and secure location.
Information that is required to be on an MIS report includes the following:
- The total number of drug and alcohol tests by the type of substance tested. Drugs that are tested include cannabis, amphetamines, opioids, cocaine and PCP
- The number of positive drug and alcohol test results by type of substance tested and reason for the test.
- The reason for the drug or alcohol test, such as pre-employment, random, post-accident, reasonable suspicion or cause, return-to-duty, or follow-up
- The number of individuals who refused testing and the type of refusal
In most cases, a smaller motor carrier company (typically less than 50 employees) is randomly chosen each year by the FMCSA to submit their MIS results. If your company is large enough, you might be required to file a MIS report to the FMCSA every year. To determine if this applies to your company, contact the FMCSA via their website.
Whether you are a company chosen at random or submit yearly, a response is mandatory. Failure to do so may result in civil penalties of up to $1000 per day that the motor carrier does not reply.
Previously, you could file an MIS report by fax or by mail. However, it is now required to create a unique sign-in through the government website and file online.
